Individual Supported Living / ISL

Individual Supported Living (ISL) services provide personalized supports that empower adults with disabilities to live as independently as possible in their own homes and communities. These services are tailored to each person’s unique needs, preferences, and goals, offering assistance with daily living skills, health and safety, and community integration. Our ISL program is centered on choice, independence, and dignity,  ensuring each individual has the support they need to build a meaningful life, maintain their home, and actively participate in their community.

Thrive Day Program

Community-Based Day Services are individualized, community focused supports that help adults with disabilities build skills, pursue personal interests, and actively participate in everyday community life. Our  Thrive Day Program is designed around the idea of a meaningful day, where people have choice, make real contributions, and experience personal growth. Through community engagement, skill building, and relationship development, individuals are supported to live fuller, more connected lives.

Home Skills Development

Home Skills Development services provide individualized support to help adults with disabilities build and strengthen the skills needed for greater independence in daily life. Supports focus on practical areas such as cooking, cleaning, budgeting, personal care, and household management within each person’s home environment. Through hands-on learning and real-life application, individuals gain confidence, increase independence, and take an active role in managing their daily routines.

Group Community Networking

Group Community Networking services provide opportunities for adults with disabilities to build social connections, explore shared interests, and engage in meaningful activities within their communities. Through small group experiences, individuals develop communication skills, foster friendships, and participate in activities that reflect their interests and goals. With an emphasis on inclusion and relationship-building, individuals are supported to feel connected, valued, and involved in community life.

Personal Assistance Supports

Personal Assistance services provide individualized support to help adults with disabilities  maintain their health, safety, and independence in daily life. Supports may include assistance with personal care tasks such as bathing, dressing, mobility, and hygiene, as well as help with daily routines. With a focus on dignity, respect, and personal choice, individuals receive the support they need in a way that promotes independence and overall well-being.
